Get back to work safely with Centaris COVID Care App!
Introducing a newly designed app to help assist Michigan businesses get back to work! We understand that safety is a priority during these stressful times. That's why we created the COVID Care App to allow employees to communicate their current COVID-19 status.
Our COVID Care App includes:
- An Employee Self-Declaration Survey
- Submittal of Test Results
- Test Results History
Centaris COVID Care App features a Self-Declaration Survey, Submitting Status Results and Latest Results page.
Employees are able to take a picture of their COVID-19 test results while reporting the date of the test, result date and test results.
The results can be displayed later if anyone requires proof of testing.
If an employee clicks “Yes” to experiencing any cold or flu-like symptoms in the last 14 days, a box featuring various symptoms will appear. The employee can then click any of the symptoms they have.
Once employees have completed the Self-Declaration Form, the survey with all of the results will be emailed. The results are not shared with any other employees aside from Human Resources.
Each completed Self-Declaration survey is stored for each employee and can be exported at any time.
