Introducing a newly designed app to help assist Michigan businesses get back to work! We understand that safety is a priority during these stressful times. That's why we created the COVID Care App to allow employees to communicate their current COVID-19 status.
Employees are able to take a picture of their COVID-19 test results while reporting the date of the test, result date and test results.
The results can be displayed later if anyone requires proof of testing.
The COVID-19 Self-Declaration Questionnaire:

When employees answer “Yes” to any of these questions, a Date of Exposure box will appear.
If an employee clicks "Yes" to experiencing any cold or flu-like symptoms in the last 14 days, a box featuring various symptoms will appear. The employee can then click any of the symptoms they have.
Once employees have completed the Self-Declaration Form, the survey with all of the results will be emailed. The results are not shared with any other employees aside from Human Resources.
Each completed Self-Declaration survey is stored for each employee and can be exported at any time.
